Authorized Access Agreement 

Selection Management

I understand that as a designated representative of , I am authorized to request and receive employment screening reports from Selection Management.

I hereby agree that I shall only request background reports on applicants for employment. I further agree not to obtain these reports on: myself, my associates or any other person except in the exercise of my official duties.

I agree to keep confidential the passwords and user identification numbers with which to access reports from Insights and will not share these numbers with other employees who do not have an official responsibility to obtain such reports.

I understand the Fair Credit Reporting Act stipulates that anyone who knowingly and willfully obtains information on an individual from a consumer reporting agency under false pretenses shall be subject to fine and/or imprisonment.

I hereby indemnify my employer, Selection Management and any reporting agencies used to obtain unauthorized reports from any actions on my part deemed unlawful by the provisions of the Fair Credit Reporting Act.
